Continuing Care in addiction And Mental Health treatment

Completing a residential treatment program is a major accomplishment. It is a key step in achieving sobriety and beginning recovery. Treatment is just the first step and your recovery will be a lifelong journey that requires attention and care, especially during the transition out of Sabino Recovery. Leaving residential treatment can feel overwhelming and isolating. Our Continuing Care Program is designed to help you take steps to transition back to your life.

It is important to set yourself up for success with the support you need. Residential treatment is an opportunity to step back from your normal life, learn new coping tools, and live without substances. Returning home to familiar people and places can be triggering. In treatment, you gain knowledge, tools, and strategies to cope with triggers. Your Continuing Care plan gives you support to practice applying what you’ve learned as you continue to heal.

Many of the things you learn at Sabino Recovery will profoundly change you. You will need time and support to understand those changes and how to bring them into your life.

Our Continuing Care Program

You will meet weekly with the Continuing Care Coordinator for interactive and educational sessions about the importance of ongoing care and work to identify your triggers. They will also help you recognize unhealthy boundaries and life patterns while working with you to set and maintain healthy boundaries. You will create a plan for creating healthy habits and building healthier support networks while rediscovering your passions. These will reinforce your recovery, improve your coping responses, benefit your employment, and provide a sense of self-worth and self-esteem.

Weekly Primary Test Therapist One-On-One

As you move through the program here at Sabino Recovery, you and your treatment team will understand what treatment modalities you respond to and benefit from. This allows you to build a plan that will incorporate these therapies into your everyday life at home. Every week you will meet with your therapist and as part of your session, you will be working on your plan for life after Sabino Recovery.

Weekly Continuing Care Coordinator One-On-One

While you are still in residential treatment at Sabino Recovery, you will meet with the Continuing Care Coordinator to discuss your progress and what transitional programs, therapies, and modalities will be best to help ensure your continued recovery. You and the Sabino treatment team will identify programs, support groups, and modalities within your means and where you intend to live. The goal is to set up appointments before your discharge and make sure you know what to expect. You will also talk with the team about your interests and goals such as social groups, volunteer opportunities, school, and hobbies to help you thrive.

Before you discharge, you will have a sustainable, recovery-focused, self-care plan that will help ensure your continued health. Recovery is not always straightforward. We will do our best to set you up for success but we can’t predict every circumstance. If you return home or to a transitional program and things aren’t working out, such as the groups aren’t right for you or you can’t make your appointments, we are here to help. We will recalibrate and help you create a continuing care plan that works for you and your recovery.
